Bracken County Nursing and Rehabilitation is the ONLY 5-star rated nursing facility in the multi-county region that makes up the Buffalo Trace Area Development District, including Mason, Bracken, Fleming, Pendleton, Robertson, and Lewis counties. We may be small, but for the Quality of Resident Care, Bracken County cannot be beat! The star rating system is designed by the Center for Medicare and Medicaid Services to evaluate the Quality of Longterm Nursing Facilities. That’s right, BRACKEN COUNTY NURSING AND REHABILITATION IS THE ONLY 5 STAR!
Of course, we do not need a report from the government (which can be seen at http://www.medicare.gov/NursingHomeCompare/search.aspx?bhcp=1). We are frequently reminded by the community members we serve. We do not often let people of Bracken County know just how well we do what we do. But, yesterday, I received the following note:
To All employees at Bracken County Nursing home,

Augusta, Ky. (August 15, 2012) — Bracken County Nursing & Rehabilitation Center is proud to announce that it has received a five-star quality rating, the highest overall score, from the federal Centers for Medicare & Medicaid Services (CMS).
Bracken County Nursing & Rehab is a 32-bed nursing community located at 5269 Asbury Road, in Augusta, Ky. It is one of 72 nursing facilities operated by Louisville, Ky.-based Signature HealthCARE LLC.
Under the five-star rating system, nursing homes are assessed in three main categories: health inspections, quality measures and staffing. The overall rating combines scores received in each of the three categories. The system is intended to help consumers, families and caregivers compare nursing homes more easily and identify areas about which they would like to learn more. The building received especially high marks based on residents’ health, physical functioning, mental status and general well being.
“We are extremely excited about being the only 5-Star rated facility in the Buffalo Trace Region. We have an exceptional staff dedicated to providing the highest quality care to both short-term rehabilitation and long-term residents,” stated facility interim administrator, Brad Wyatt.
